Giving by Mail - Donations may be mailed directly to: Riverview Health Care Foundation 410 Dewey Street P.O. Box 8080 Wisconsin Rapids, WI 54495

Giving in Person - The Foundation staff would be delighted to meet with you personally to discuss your donation and answer any questions you may have. Feel free to call the Foundation office (715-421-7488) to set up an appointment, or simply stop by the Foundation office during business hours (Monday, Tuesday, Thursday, & Friday from 8:00 to 4:00).

Giving Online - For your convenience, you can donate to Riverview Health Care Foundation via our secure website.

Types of Gifts - Riverview Health Care Foundation receives many types of gifts, including: MEMORIALS - gifts given in memory of a loved one; HONORARIUMS - gifts given in honor of someone; OUTRIGHT GIFTS OF CASH OR PROPERTY; CAPITAL CAMPAIGNS - gifts given or pledges made toward a specific project; and PLANNED GIFTS - bequests given through one's will.

Bequest Language

To leave a tax-deductible gift in your will to Riverview Health Care Foundation for the benefit of Riverview Hospital Association and community health care initiatives, consider the following:

I leave and bequest unto Riverview Health Care Foundation, the sum of $_______, or ____ percent of my estate (or specific securities or other property). Said Foundation is a nonprofit 501(c)(3) corporation (Federal ID number 39-1509239), organized under the laws of the state of Wisconsin, with administrative offices at 410 Dewey Street, Wisconsin Rapids, WI 54495. This gift is unrestricted for general Foundation purposes.
